Scholarship Applications:

2012 Applications available March 30

Eligible Applicants 

Any boy or girl living in the Capital District and surrounding areas who has shown a positive attitude toward school and other areas of his or her life.  The focus is on families with low income levels with exceptions for larger families.

Application Process

1. Contact Fast Break for an application:
 lhicks@pva.net or call Lisa Hicks @ 881-1670 x-3.
2. Complete attached application form and return to the Fast Break Fund.  Please be sure application is signed by the child’s parent or legal guardian.
3. Along with application please send a copy of last semester’s report card.  We are looking at attitude and effort.
4. Parent, child, or teacher must write a short paragraph explaining why they are deserving of this scholarship.

Any applications arriving after June 1^st will NOT be considered for a scholarship in 2012.
