Scholarship Application 

Eligible Applicants 

Any boy or girl living in the Capital District and surrounding areas who has shown a positive attitude toward school and other areas of his or her life.  The focus is on families with income levels under $25,000 with exceptions for larger families.

Application Process

1. Contact Fast Break for an application form kdzikas@pva.net or Call Kris Dzikas @ 881-1670 ext.2.
2. Complete attached application form and return to the Fast Break Fund.  Please be sure application is signed by the child’s parent or legal guardian.
3. Along with application please send a copy of last semester’s report card.  We are looking at attitude and effort.
4. Parent, child, or teacher must write a short paragraph explaining why they are deserving of this scholarship.
